For all of you who I know in real life (or just those of you who want to be in my address book/phone list/birthday calendar), please fill this out for me. It's secure through Google Forms and I'll be the only one to see the results. This form is to help me update things for both family and friends and to help me do it very easily without having to get in direct contact with every single person to make sure it's all up to date.
